About the PCP
Some insurance plans require a PCP or Primary Care Physician. You can find out whether or not your plan does, by checking for PCP on the front of your card or giving the member services number on the back of your card a call. If you find your plan does, give member services a call and add either Dr. Kalanksy, Dr. Uythoven, or Teresa Guthrie as your Primary Primary Care Physician (you'll have to do this for each child). You can see any physician in the practice regardless of which you choose as your PCP. It is important to select your Primary Care Physician because if it is unlisted or incorrect your visit costs will not be covered by the insurance and we will not be able to generate referrals should you need them.
Annual Physicals/Well Visits
Every child from age two and up should have a physical each year. Each insurance plan has different requirements as to when your child is eligible for a physical. Some Insurance plans require a birthday to occur between well visits, some require exactly 365 days to pass between visits, and some require you be in a different calendar year for each physical. If you have physical outside of these limitations your insurance company will not cover it and you will be responsible for the cost of the visit. It is confusing business, we know! Its best to consult your plan benefits booklet you received upon enrolling or give member services a call and ask them what the limitations are for your plan.
